Key responsibilities: Complete timely and appropriate assessments that consider the individual's physical, emotional, and environmental needs. Recommend tailored interventions such as equipment, adaptations, rehabilitation, re-housing options, or community-based support. Support individuals to maximise independence through tailored rehabilitation programmes, practical support, and the effective use of equipment, adaptations, and assistive technology to improve safety, mobility and quality of life. Requirements: Must have a Diploma/BSc in Occupational Therapy and must have current HCPC registration. Experience of working in a community setting undertaking home visits and assessments. Experience of manual handling assessments and care planning.
