Vertis Health is a fast-growing provider of community-based healthcare services to NHS patients within South Worcestershire. The organisation utilises clinical expertise within the local NHS environment and efficient administration pathways to provide more convenient and accessible services, aiming to relieve growing volume pressures on local hospitals and GP practices.
Salary: £46,148 to £52,809 a year (pro rata for part-time employees, dependent on experience).
Responsibilities
* Provide advice and support on prescribing and medication-related issues for care home residents as part of a multidisciplinary team.
* When clinically appropriate, implement and action changes to patients' medical prescribing records as directed by primary, secondary or community care, operating at all times within professional limits.
* Provide expertise in clinical medicines review and address public health and social needs of these patients.
Requirements
* Experience in clinical medicines review and prescribing within a primary or community care setting.
* Ability to work as part of a multidisciplinary team and to implement changes safely and effectively in line with professional standards.
* Commitment to delivering high-quality, patient-centred care and to promoting inclusion and reducing inequalities in health outcomes.
