An exciting opportunity has arisen for a Band 6 to cover maternity leave for 10 month working within the established Respiratory Specialist Team playing a key role in this varied role organising the smooth running of the virtual ward (supporting patients in their own home), nurse led clinics, front of house A&E and acute wards assessing acutely unwell Respiratory patients, ensuring that all Respiratory patients within the case load receive the highest standard of clinical care. .
You will demonstrate a high level of expertise and contribute to developing the Respiratory Specialist team both in secondary and primary care settings, providing specialist advice, presenting education/health promotion to both patients and staff. The post involves covering a 7 day service This post is 25 hours and will cover 3 days a week.
The post holder must be highly motivated and dynamic and able to make decisions independently and within the team. To manage a case load of respiratory patients in the hospital setting and in the community
The post holder will support with the training and assessment of student nurses, always demonstrating the Trust Values and Behaviours for Nursing & Midwifery.
Main duties of the job
Clinically assessing & reviewing patients in an acute setting and in their own homes
·To ensure the patient and carer are involved in their care where possible, ensuring consent and agreement is obtained
·Managing a caseload of patients referred to the speciality whilst working as part of the multi-disciplinary team, delivering individualised and personalised direct patient care
·Participate in the promotion of patient centred care, and establish and maintain a supportive relationship with the patient and their family during the period of assessment, treatment and after care
·To ensure specialist skill and knowledge is maintained to provide a wide range of treatments and demonstrate flexibility in developing new extended clinical skills to meet service requirements
·Providing a high standard of holistic care for a group of patients encompassing their physical, psychological & social needs
·To undertake strategies to reduce avoidable harm, including exemplary infection prevention and control practices
·To ensure compliance with legislation and local guidance relating to safeguarding, DOLS, MCA and MHA, representing the Trust with transparency and objectivity
·Considers and interprets all available information and makes justifiable assessment of the individuals health & well-being, related needs and risks, explaining the possible outcomes to those concerned
·Acts as patients advocate respecting patients dignity wishes & beliefs involving them in decision making about their care/treatment
·Ensures that Nursing care is delivered with compassion
·Liaises with allied health professionals and other agencies to ensure that care is delivered in a timely and organised way
