This role requires providing expert clinical case management, using advanced nursing skills, to patients of all ages. Responsibilities include assessment, treatment, discharge, or onward referral to secondary care, ensuring appropriate advice and treatment. The nurse will lead in delivering high standards of care in partnership with the multidisciplinary team, working to improve care standards.
The nurse will develop and deliver care programs for a substance misuse service within a prison environment. Responsibilities include initial assessment, planning, implementation, and ongoing care for prisoners with substance dependence or related impairments, adjusting care daily, including medication management in collaboration with medical staff.
The nurse will provide comprehensive discharge and follow-up services, managing their caseload within an integrated team providing specialist drug problem services at HMP Edinburgh. The role involves building relationships with statutory and voluntary organizations and promoting harm reduction philosophies.
The nurse will manage a caseload of prisoners with complex needs related to substance misuse, with autonomy to discharge prisoners from caseloads and provide ongoing support, education, and advice on healthcare management.
