Senior physical health check coordinator

Swindon
SECOND STEP
Coordinator
£25,000 - £40,000 a year
Posted: 21 September
Offer description

Second Step is a leading mental health charity in the Southwest offering housing, support and hope to thousands of people with mental health and other support needs.

We aim to inspire hope and deliver change for everybody and every community we work with.

Do you want to support people with severe mental illness to improve their physical health?

Can you build trusting relationships that encourage people to take positive steps for their wellbeing?

Are you ready to work in a dynamic role combining autonomy with teamwork in a recovery-focused service?

If so, you could be the person we are looking for.

About the Service

This role is part of the Mental Health and Wellbeing Partnership across Bath and North East Somerset, Swindon and Wiltshire (BSW), which includes Second Step, Alabaré, Missing Link and Nilaari. Together, we deliver community-based, trauma-informed support that places people at the heart of mental health and wellbeing services.

The Senior Physical Health Check Coordinator supports individuals with severe mental illness, such as psychosis, bipolar disorder and schizophrenia - to access annual NHS health checks and make positive lifestyle changes based on the results.

The role is hybrid with an expectation of working the majority of that time out of GP surgeries and a day in the office. It might also require meeting people at or in their homes.

About You

You will:

* Work directly with individuals on the SMI register to help them understand, attend and follow up on their physical health checks.
* Support clients to access community services such as walking groups, smoking cessation programmes or other wellbeing resources.
* Collaborate closely with GPs, Primary Care Networks and local voluntary partners to coordinate access to health and wellbeing services.
* Maintain accurate client records and share insights to improve service delivery.
* Deliver care in line with trauma-informed principles, valuing lived experience, equality, diversity, and inclusion.

You'll need:

* At least one year's experience supporting people with mental health or complex needs.
* Strong knowledge of health, wellbeing, and safeguarding practices, including how to support people experiencing severe mental illness.
* Excellent communication and interpersonal skills, with the ability to build trust and rapport quickly.
* Strong organisational and IT skills to manage appointments, data and reporting.
* A commitment to recovery-based, person-centred, and culturally sensitive support.

We especially welcome applicants with lived experience of mental health challenges and from underrepresented communities.

Second Step offers an excellent annual leave package, access to our Employee Assistance Programme, and a comprehensive induction with ongoing learning and development opportunities.

So…if you think this describes you, what are you waiting for?

Why not apply now?

Please be aware that we are currently recruiting for additional part-time roles within the area if you are interested in applying for other opportunities as well to make up a full-time role.

Closing date: Wednesday 17th September 2025 midnight

Interviews: TBC

The ability to travel to several appointments daily is essential.

For more information and to apply please see our website Second Step is committed to Equal Opportunities and welcomes applicants from all sections of the community and from people who have personal experience of using mental health services. Unfortunately, Second Step is unable to sponsor applicants from outside the UK.
