What you will do:
* Conduct assessments using Dialog and complete risk assessments with working-age adults.
* Develop collaborative care plans and deliver a mix of generic and profession-specific interventions in both individual and 1:1 settings.
* Cover duty on average once a week, supporting triage activities and guiding rotational Band 5 Occupational Therapists and students on placement.
* Participate in the duty rota (average once per week) to support triage, risk management, and urgent assessments.
* Liaise with other services, including primary care, secondary care, social care, and voluntary sector organisations, to ensure seamless care pathways.
* Maintain accurate and up-to-date clinical records, adhering to professional and organisational standards.
* Contribute to service development, audit, and quality improvement initiatives within the team.
Requirements:
Hold HCPC registration with at least 2 years of experience.
Being a car driver is preferable to aid in travel and commute during your duties.
