What you will do:
* Work as an integral member of the clinical team, including physiotherapists, occupational therapists, and therapy assistants within community therapy services.
* Independently manage a caseload that includes highly complex patients.
* Provide specialist assessments for adults with physical dysfunctions or disabilities resulting from injury, illness, surgery, or chronic disease across various community settings, including crisis response units, intermediate care beds, rehabilitation wards, clients’ homes, and care homes.
* Assess, implement, and evaluate individual care programmes on an ongoing basis.
* Collaborate with a multidisciplinary team, including consultants, GPs, nursing staff, social workers, and other allied health professionals, regarding patient management and discharge planning.
* Deliver clinical education and training to staff within community services as required.
Requirements:
* HCPC registered Physiotherapist.
* Full UK driving licence and access to a vehicle.
* Experience managing complex rehabilitation caseloads.
* Strong multidisciplinary team-working and communication skills.
