Main duties of the job
The post holder will join the team in the Specialist Children's and Young People's Services in Newham to work as the Transition Care Community Nurse Specialist across the Community Children's Nursing Service (CCNS) and the Child Development Service (CDS) providing Transition Care advice and clinical support to young people aged 14-19 years with Special Educational Needs and Disabilities (SEND) and related complex health needs.
We are looking for an experienced innovative clinical nurse with extensive experience managing the care of children with complex medical needs and learning disabilities.You will be joining a service which aspires to provide care of the highest quality, in collaboration with those who use our services. We embrace continuous improvement and learning and as a Specialist Nurse you will be instrumental in leading the service to think differently, be innovative, and give everyone, at every level, the skills they need to lead change.
The post holder will manage a clinical caseload to include young people with SEND and related complex health needs aged 14-19 years, ensuring a smooth transition to adult health services.

* RSCN/ RN Child Branch
* Evidence of continued professional development
* Educated to Degree level in nursing or other relevant field.
