3 Circles Care is looking for a motivated and compassionate Team Leader to help oversee our Domiciliary Home Care services. You’ll work closely with our Registered Manager and RI to support and supervise care staff, deliver high-quality, person-centred care, and ensure our services meet and exceed expectations.
You will be case managing service users and liaising with families, healthcare professionals and other stakeholders to ensure that the best care plans are created and delivered.
You will assist with the following:
* ·assisting with scheduling;
* ·carrying out initial prospective service user assessments;
* ·carrying out care plan reviews;
* ·administration, auditing and recording of medication;
* ·carrying out and reviewing risk assessments;
* ·staff monitoring, spot checks and compliance;
* ·emergency management: be prepared to manage emergencies calmly and efficiently;
* ·take part in out of hours emergency on-call rota. For example, either covering calls directly or indirectly when care staff are sick or absent;
* ·manning the on call mobile once a week and as otherwise required; and
* ·covering staff holidays as well as sickness and absenteeism.
Key Responsibilities Include:
* Supporting and supervising care/support staff in the community;
* Completing care plan audits, competency checks, and face-to-face supervisions;
* Assisting with the assessment and setup of new customer care packages;
* Delivering personal care when required and modelling best practice;
* Monitoring and maintaining compliance with CIW standards and KPIs;
* Contributing to recruitment, induction, and mentoring of new care staff;
* Participating in the on-call rota and covering shifts when needed;
* Maintaining accurate records and electronic systems; and
* Acting as a link between service users and the office, promoting professionalism and respect.
About You
* QCF 3 in Health & Social Care desirable;
* Previous experience desirable;
* Proven ability to lead and supervise others;
* Ability to keep calm under pressure;
* Confident to work alone as well as a part of a team;
* Approachable;
* A team player;
* Able to work; days, evenings, weekends and Bank Holidays as necessary;
* Organised with excellent communication, numeracy and literacy skills;
* Not under a disciplinary or safeguarding investigation
* Clear DBS;
* Full driving license and use of a car for work.
