Join Our Team as a Nurse Practitioner!
General Summary:
Are you ready to make a meaningful impact in the lives of our participants? As a Nurse Practitioner at Element Care, you will be an integral part of our PACE Interdisciplinary Team. Your expertise will help us plan, implement, and evaluate care plans that align with the PACE model of care. Collaborating closely with our Medical Doctors, you will provide comprehensive primary care in a supportive and dynamic environment.
Essential Responsibilities:
* Conduct initial examinations for all new participants and quarterly check-ups during their first year of enrollment.
* Evaluate participants during episodic visits, manage acute illnesses, and follow up on hospitalizations.
* Make nursing home visits as required by state regulations and as needed.
* Provide home visits for participants who are homebound, whether acutely or chronically.
* Participate in After Hours On-Call Coverage.
* Deliver health care education to patients, families, caregivers, and fellow team members.
* Adhere to the Element Care NP/MD Collaborative Practice Guide.
* Complete accurate and timely documentation, both onsite and offsite.
* Ensure fiscal responsibility while adhering to established budgetary constraints.
* Maintain the highest standards of care in all interactions with clients.
* Pass a fit test for mask requirements; facial hair must not interfere with a tight seal.
* Engage in frequent local travel.
* Perform other duties as assigned.
Job Specifications:
* Master of Science Degree in Nursing (MSN).
* Current Board Certification as a Nurse Practitioner in Massachusetts.
* Massachusetts and DEA Prescription Privileges.
* Credentialed for Element Care affiliate Long Term Care Facilities.
* Current CPR Certification.
* A minimum of one (1) year of experience working with a geriatric population.
* Strong communication skills to convey information clearly and succinctly.
* Ability to lead independently while collaborating effectively with others.
* Team-oriented mindset with a focus on supporting colleagues.
* Attention to detail, self-starter attitude, and timely task completion.
* Quickly recognize and address situations before they escalate, implementing effective solutions.
* Covid vaccinations are required.
