Job Overview

Successful candidate will support the DN Team Leader with leadership initiatives including caseload and staff management within the District Nurse Service structure. Depending on the demand of service, the post holder will support new initiatives including post‑Covid business as usual. They will complete home visits to provide nursing care and complex patient management and support. They will manage a clinical caseload that meets the needs of all patients across the local landscape and network, providing unlimited opportunities to develop clinical skills and test leadership styles. The role requires weekend work on a rotational basis to support and coordinate weekend working across the locality. Previous applicants within last 3 months need not to apply.

Detailed Job Description & Main Responsibilities

* Using clinical judgement and risk assessments to keep the people using our services as safe as possible.
* Safeguarding people by recognising and responding when an adult or child might be at risk from abuse, recognising own limits, asking for help, and escalating concerns when necessary.
* Escalating safety concerns and acting as effective advocates for those who use our services.
* Being open and transparent about their own practice.
* Supervising the work of others.
* Reflecting on everyday practice to identify areas where improvements in safety or quality can be made.
* Working with others to create a culture of continuous improvement.
* Maintaining accurate, legible, comprehensive records.
* Maintaining compliance with mandatory training requirements.


Clinical Outcomes

A Band 6 Deputy District Nurse often works with people who have complex needs and contributes to the effectiveness of services by:

* Being a confident, autonomous practitioner who takes responsibility for the care they give to patients and also oversees the work of others in the team, including less experienced nurses and staff without a professional qualification.
* Providing skilled, evidence‑based nursing care that adheres to agreed policies and procedures.
* Working with patients and families in all stages of the care planning process, including assessing risks and needs.
* Acting as patient advocates within the multidisciplinary team and overseeing the work of others to ensure that they are responding to the needs of patients and providing clinically effective care.
* Working as an autonomous practitioner and taking responsibility for the care they give to patients while also working as a team member.
* Contributing to creating and maintaining high‑performing teams by.
* Communicating well with all members of the team.
* Understanding their role in the team and how they help the team achieve its objectives.
* Reflecting on their own practice regularly and encouraging the whole team to reflect on their practices in handovers and team meetings.
* Encouraging the team to learn from adverse events or respond to data analysis from audits or benchmarking exercises and implement sustainable initiatives that improve clinical outcomes.


Qualifications – Essential Criteria

* Currently registered NMC Adult Nurse.
* Evidence of relevant Continuous Professional Development.
* Numeracy skills required for the safe administration of medicines.


Qualifications – Desirable Criteria

* Mentorship qualification.


Experience – Essential Criteria

* Previous experience as a Band 5/6 community nurse or equivalent level.
* HR skills training (e.g., recruitment, managing absence).
* Care planning: ability to assess patients, develop, implement and evaluate a care plan.
* Empowering and working in partnership.
* Knowledge and understanding of Safeguarding/Advocacy.
* Knowledge and understanding of the Mental Capacity Act and Deprivation of Liberty Safeguards.
* Knowledge and understanding of the Mental Health Act.
* Manual handling of patients.
* Obtaining consent.
* Medicines management and administration.
* Infection control and aseptic non‑touch technique.


Experience – Desirable Criteria

* Supervision, line management and appraisal training.


Other – Essential Criteria

* Ability to understand and implement the Equal Opportunities Policy at a level appropriate to the job.


Other – Desirable Criteria

* Valid UK driving licence.
* Access to a car or other motor vehicle for work purposes.


Skills And Abilities – Essential Criteria

* Ability to communicate with the multi‑disciplinary team, present cases, note actions, update others and use handovers effectively, speaking face‑to‑face, by telephone, and writing letters and reports.
* Maintaining accurate, timely, concise and legible clinical records, both electronic and handwritten, in accordance with CNWL policies.
* Ability to discuss sensitive issues with people in a supportive manner while challenging others assertively when necessary.
* Ability to maintain electronic patient records.
* Clinical skills specific to service: catheterisation, wound care, palliative care & end‑of‑life care, IV administration, injections, Doppler assessments, compression bandaging, diabetes management, venepuncture.
* Coordinate work allocation and shifts (if appropriate).
