About the Role
We are recruiting a Band 6 Community Dietitian to join the Community Adult Rehabilitation Service within CNWL.
This is an excellent opportunity to work as part of a supportive multidisciplinary team, delivering specialist nutritional support to adults within their own homes, care homes, and intermediate care settings.
Key Responsibilities
* Provide specialist dietetic assessment and treatment for adults requiring nutritional support, including Home Enteral Tube Feeding (HETF)
* Deliver evidence-based nutritional interventions for patients who are malnourished, at risk of malnutrition, or enterally fed
* Assess and calculate nutritional requirements, including food fortification, oral nutritional supplements, and enteral feeding regimens
* Provide dietary counselling and support behaviour change to patients, families, and carers
* Make recommendations to GPs regarding appropriate prescribing of oral nutritional supplements and enteral feeds
* Participate in MDT meetings, discharge planning, and collaborative community care pathways
* Maintain accurate clinical documentation and caseload management
* Support training and education for healthcare staff, carers, and students
* Contribute to audit, service development, and quality improvement initiatives
Essential Requirements
* Degree or postgraduate qualification in Dietetics
* HCPC registration as a Dietitian
* Experience working within community or adult rehabilitation settings
* Knowledge of nutritional support, including oral nutrition support and enteral feeding
* Strong communication, clinical reasoning, and organisational skills
* Ability to work independently and manage a clinical caseload effectively
* Full UK driving licence and access to a vehicle for community visits
Desirable
* Experience in Home Enteral Tube Feeding services
* Experience supervising students or support staff
* Knowledge of rehabilitation and long-term condition management
* Experience contributing to audit or service development projects
