We are pleased to offer an exciting opportunity for a highly skilled clinical practice leader to join us in the North West HOPE(S) team as a Specialist Practitioner.

Are you passionate about and committed to reducing restrictive practices? Are you looking for a role with lots of variety? Are you looking for experience of working in the wider health and social care system whilst using your clinical skills working with people in Long Term Segregation?

Due to new investment and extension of funding we have 2 x 2 year secondments available:

HOPE(S) Practitioners work directly with people who are referred to the programme and their teams, using the HOPE(S) model and clinical skills and experience to reduce restrictive practices, end or reduce time in segregation, to provide clinical input into discharge planning and to clinically support commissioners and commissioned teams and providers to avoid admission and escalation to secure services.

The HOPE(S) Clinical Model was developed by Mersey Care NHS Foundation Trust and the trust hosts this work in the North West,

Please note-the successful post holder will work in direct clinical care environments across North West England and occasionally support North West funded patients elsewhere

To provide strategic clinical leadership and innovation using the most up to date and evidence-based theory and practice with a specific focus on the application of the HOPE(S) clinical model of care to reduce long term segregation and innovative strategies to improve clinical practice and reduce restrictive practices with children and young people, adults with autism and/or a learning disability and those with mental ill-health across Integrated Care Boards (ICB’s) and Lead Provider Collaboratives in partnership with Mersey Care NHS Foundation Trust.

This secondment provides a rare opportunity to develop specialist clinical skills working into multiple health and social care providers and will provide with development opportunities which they can utilise on return to their own Trust

Detailed job description and main responsibilities

To support Prospect Lead Provider Collaborative in the delivery of clinical quality improvement and innovation in relation to reducing long term segregation and other forms of restrictive practice. To provide Clinical and Practice leadership for the HOPE(S) programme. To deliver key outcomes on this portfolio to the Programme Steering Group.
To offer expert clinical advice and chair meetings with organisations within Prospect Lead Provider Collaborative.To offer expert clinical advice for service users, carers, staff and other professionals. To demonstrate advanced competence in highly complex decision making, assessment and the management of clinical needs within
In Patient Services across Prospect Lead Provider Collaborative. To work in partnership with service users, carers and other practitioners to ensure effectiveness of outcomes.


Person specification


Qualifications

* Professional Registration e.g. Registered Nurse Mental Health
* Educated to or working towards Masters degree or equivalent level, Or Post-graduate doctoral level training in clinical psychology accredited by the BPS, or its approved equivalent and professional registration with the HCPC
* Demonstrable experience in clinical practice at a senior level
* Evidence of post registration study / continuing professional development at masters level or equivalent


Knowledge/Experience

* A demonstrated understanding of Equality and Human Rights
* The post holder would be expected to have extensive abilities in delivering innovation and quality in clinical practice in a range of specialties, demonstrating leadership skills, change management expertise and the delivery of clinical developments that have tangible results for patients
* Experience of Consultancy on complex cases and service improvement
* Will have experience of promoting clinical effectiveness & standards setting via audit & governance frameworks
* Will also be able to demonstrate training and teaching expertise in applied settings
* Will be service user focused with a track record that includes examples of clinical practice that have required collaborative interdisciplinary work to improve adult services
* Will require an in-depth knowledge of clinical care with significant evidence of post-registration development activity
* Must be able to demonstrate a commitment to service user involvement
* Be able to work autonomously in a self-motivating style
* Computer literate with a working knowledge of standard formats including spreadsheets and data management
