The role involves working directly with adults aged 18+, including those with learning disabilities, physical disabilities, long-term conditions, dementia, drug and alcohol-related needs, homelessness, hoarding behaviours, and individuals transitioning into adulthood or older age. You will also support young carers and adult carers, ensuring they can continue their caring role safely and sustainably.
