What You’ll Do:
* Provide tailored care in both day service and community settings, responding to complex and evolving needs.
* Maintain accurate and auditable care records including daily notes, risk assessments, and incident reports.
* Act as a keyworker for individuals, contributing to care plan reviews and wellbeing reports.
* Recognise and escalate changes in health or behaviour to ensure timely interventions.
* Deliver personal care with dignity and respect - including bathing, dressing, toileting, and mobility support.
* Administer prescribed medication safely and accurately, including specialist preparations.
* Foster meaningful communication and emotional wellbeing through social engagement and activities.
* Respond swiftly and effectively in emergency situations, including administering first aid and rescue medication.
* Collaborate with families, carers, and professionals to ensure holistic, person-centred care.
What We’re Looking For:
* Experience in care or support work (preferred but not essential - training provided).
* Compassionate, reliable, and proactive individuals.
* Strong communication and record-keeping skills.
* Ability to work independently and as part of a team.
* Willingness to undertake personal care and medication administration.
