We are looking for a dedicated and compassionate colleague to join our Paediatric Critical Care Unit team. This role is vital in coordinating continuing healthcare support for children with complex, long-term health needs.
You will be an important liaison between PCCU, the Long-Term Ventilation Unit, community services, families, external agencies, and the Integrated Care Boards to ensure seamless care pathways and discharge planning.

You hold relevant qualifications such as RSCN/RN (Child) and HDU/ICU training, alongside a management qualification and registered non-medical prescriber status (or a willingness to work towards these). You bring strong communication skills and a collaborative spirit.
Main duties of the job
Reporting to the Lead Nurse (PCCU), your responsibilities will include:
Lead assessment and planning of continuing care packages for children with complex or long-term conditions.
Ensure safe discharge from PCCU to home or the LTV Unit.
Be the main contact for families and multidisciplinary teams.
Conduct detailed health needs assessments aligned with national continuing care frameworks.
Collaborate with community nursing, social care, education, ICB, and other relevant agencies.
Provide communication and guidance to parents/carers about available support and funding.
Advise PCCU staff on continuing care processes and policies.
Deliver training and advice to families and community teams on managing complex care needs.
Mentor and support junior staff in understanding continuing care pathways.

Qualifications and Training
* RSCN/RN(Child)
* HDU Course / ICU Course
* Teaching and Assessing in Clinical Practice
* Degree in relevant subject
* Management Qualification
* Registered non-medical prescriber or a willingness to work towards these
Experience
* Significant PCCU experience or HDU team leading experience
* Teaching and Mentorship qualification
* Basic management course
* Experience of managing a PCCU
* Line management

* Knowledge of the ventilatory care needs of the highly dependent child
