Hi, my name is Joanne White, we have the rare opportunity of 2 Senior Clinical Practitioner 12 months fixed term posts. One full time and one parttime to cover maternity leave and we would welcome applications from either Registered Nurses, Occupational Therapists or Social Workers.
The team provides support and treatment to service users in their own homes who are over the age of 65 and are experiencing the onset or relapse of severe mental distress.

We are a dedicated and compassionate team who encourage and support each other to learn, develop and grow. In recent years staff have developed their skills within the team to become Non-Medical Prescribers and a Trauma Informed Personality Disorder Clinician.
The service operates across 7 days a week between the hours of 8.00 am and 8.00 pm you will be expected to work 11.5‑hour shifts for 3 weeks of the month and 4 shifts for one week of the month (full time).
At the time of advertising, this role does meet the minimum requirements set by UK Visas and Immigration to sponsor candidates to work in the UK.
Visa sponsorship under the Health and Care Worker visa route can only be considered if the prorated salary meets the minimum threshold of £25,000, as required by the UK Visas and Immigration.
Main duties of the job
The successful practitioner will work within the remit of the band 6 Senior Clinical Practitioner job description.
You will be undertaking crisis assessments and formulating plans to support the service users to safely remain at home. You will require experience or understanding of positive risk taking and be able to complete documentation to support this. Where it is deemed unsafe to provide continued support at home you will be an integral part of acting as a gatekeeper for admissions to the ward.
A key element of your role will be your ability to work both autonomously when making decisions in the community but also sharing knowledge of the whole team caseload with the multi‑disciplinary team.
You will need to remain calm and resilient in a busy team demonstrating compassion in often highly stressful situations. Aside from this you will be well supported by the leadership team and colleagues.
You will require an interest in Older Peoples needs and understand the need for a holistic approach to their care.
You will have strong and effective communication and engagement skills with service users, family, friends and carers.
Being a car driver is essential as the team covers a large geographical area.

Job responsibilities
JOB SUMMARY
Has continuing responsibility for the provision of skilled interventions to people with mental health problems in the community. Works within a multi‑disciplinary team as part of the Intensive Home Based Treatment Team and in the role of Senior Mental Health Practitioner for the Mental Health Liaison Service (if applicable to the business continuity of services).
Key Result Areas
* Management of team caseload in line with C.P.A. procedures and risk management protocols.
* Provision of leadership to the Intensive Home Based Treatment Team in respect of psycho‑social interventions to people and their carers in line with national and local practice standards e.g. NICE.
* Supervision and support of other team members.
* Participation in service development and clinical governance structures.
Professional
* Assesses needs and identifies problems relevant to the care of people referred.
* Devises a plan of care in partnership with the person and carers.
* Implements the planned programme of care to ensure a high standard is achieved.
* Reviews the effectiveness of the care provided and where appropriate initiates any action.
* Advises and supports the person and carers to promote health and well‑being and to prevent illness.
* Recognises situations that may be detrimental to the health and well‑being of the person and initiates action that may be required.
* Establishes and maintains satisfactory communication with all agencies involved in the person’s care in both hospital and community.
* Functions as a member of a multi‑disciplinary team.
* Management of enquiries and direct referrals of people in accordance with Enhanced team operational Policy and participates in the team 24 hour duty rota including on call system (if applicable to the business continuity of the service).
* Maintain professional registration and demonstrate commitment to statutory codes of practice and continued professional development requirements.
* Responsible for acting up in the absence of the team leader.

Qualifications
* RMN Current NMC registration
* Social Worker/Occupational Therapist-current Social Work England/HCPC registration
Experience
* Can demonstrate skills in assessing and managing people experiencing complex mental health problems
* Experience of supporting and supervising colleagues/students/trainees
* Experience of using IT systems to support, record and monitor clinical activity
* Can demonstrate skills in assessing and managing risk in a variety of settings
* Proven ability in assessing the needs and strengths of people and their carers Experience of delivering a range of therapeutic interventions
* Working within an integrated multi‑disciplinary environment and co‑working with other teams and professions within team meetings, case conferences and professional meetings
* Ability to support colleagues with day‑to‑day caseload management including prioritisation of responsibilities in times of high clinical demand
* Ability to promote and develop links with other professionals for e.g practitioners within long term conditions, primary care
* Evidence of knowledge and application in practice of national guidance and standards, e.g. NICE, CQC, CQUINs
* Experience of working holistically with older adults in a bio psycho‑social model of care with emphasis on physical ill health that impacts on mental wellbeing
* Experience of working in a crisis/resolution model.
