Human Services Department is looking to hire a part-time Information and Referral Specialist/ C-EMS Coordinator. This position is Grant funded and not in a Union. The Information and Referral Specialist/C-EMS Coordinator will be under the supervision of the Director of Human Services.
Essential Functions:

* Intake, Coordination and scheduling of the Community EMS program in conjunction with other team members and the director.
* Provide in-person and telephone consultations to older adults and their families, including resource information, care planning ideas for both immediate and long term planning, problem solving and education about issues of aging. Provide information to older adults on resources and services available to them, making appropriate referrals to partner agencies and city departments with approval from elder.
* Scheduling of weekly/biweekly/monthly home and office visits, wellness calls, etc. with clients, family or caregivers as appropriate.
* Completion of department paperwork for each individual as well as the documentation of all interactions via the department’s specific software. Responsible for maintaining client files and confidential records.
* Completion of assistance program applications, i.e., fuel assistance, SNAP, housing as needed.
* Consultation with management on client case management needs; referrals to appropriate services.
* Attendance at supervision meetings and/or more frequent supervision conferences if needed. Attendance at informational meetings or in-services as assigned.
* Assist with crisis interventions, inclusive of filing Elder at Risk Reports when appropriate. Must have knowledge of the Massachusetts General Law regarding Mandated Reporting.
* Acquainting oneself with Department of Human Services/Taunton Council on Aging office procedures.
* Knowledge of specific services offered in the community to older adults. Maintain up to date program eligibility requirements.
* Develop and maintain appropriate professional relationships with community resources.
* Must be knowledgeable of client confidentiality and professional boundaries.
Education, Training, and Experience:
Associate’s Degree in the field of Human Services preferred; must have three to five years’ experience working in the field of human services, preferably with the older population. Must have excellent communication and active listening skills and be able to establish and uphold appropriate professional boundaries. Must be able to recognize the need for case management and also be aware if additional support services are needed. Knowledge and understanding of client confidentiality required. Must be able to work independently and must have use of own automobile. Bilingual candidates are encouraged to apply.
Hourly Wage: Part Time, $23.00 per hour, 19 hours per week
