Job within Renal Services Transplant Team, caring for post‑renal transplant population of 750+ patients. Ideal for experienced Staff Nurse or Band 6 Nurse seeking a challenging role.
Responsibilities
* Provide high standard of care to complex patient group; support outpatient visits and nurse‑led clinics at Salford Royal and outreach.
* Assess and triage ongoing complicated conditions, leading long‑term care.
* Educate and support healthcare professionals, patients and significant others on the patient journey to enhance quality of life.
* Collaborate with multidisciplinary and multi‑agency teams, influence and embrace change; maintain strong communication.
Qualifications
* Experienced Staff Nurse or Band 6 Nurse with renal experience at Band 5 or higher.
* Evidence of multidisciplinary and multi‑agency partnership working, influencing and embracing change.
* Non‑medical prescribing qualification desirable; training available if needed.
* Valid car driver licence essential for outreach clinics in Oldham, Bolton and Wigan.
