Job Summary
This is a community‑based Band 6 Occupational Therapist role at Leigh Moss Community Hub, Leigh Moss Hospital, Liverpool L14 5NX. The post holder will travel regularly between multiple sites to deliver mental health and dementia care across the Liverpool and Kirby footprint. Working hours are Monday to Friday, 9 am‑5 pm.
Main Duties of the Job
* Effectively manage a caseload of clients with complex needs as an autonomous practitioner, using evidence‑based, client‑centred principles to assess, plan, implement and evaluate interventions.
* Provide leadership for junior staff, including supervision, appraisal and support.
* Maintain close links with other team members and liaise with external agencies.
* Contribute to the maintenance and development of the Trust OT service, including participation in team planning, quality improvement and service governance.
* Support the clinical team in delivering multidisciplinary care, ensuring treatments meet best practice standards.

Job Responsibilities
* Identify OT goals and plan individual or group interventions using specialised mental health assessment tools.
* Monitor, evaluate and modify treatment to ensure effectiveness and progress.
* Provide training and advice on lifestyle changes and adaptations to clients’ social and physical environments.
* Assess occupational needs of a defined client group and establish appropriate treatment programmes.
* Provide clinical advice, expertise and leadership within the team.
* Coordinate care for a defined group of service users, acting as care co‑ordinator when required.
* Ensure assessment, planning and evaluation of care plans meet individual needs and comply with best practice.
* Maintain and document accurate clinical records, adhering to professional and Trust standards.
* Use communication skills to convey information sensitively to service users and families.
* Apply national guidelines, legislation and Trust policies in all aspects of service delivery.
* Lead on risk management, crisis response and serve as a point of contact for service user concerns.
* Support staff in the application of the Mental Health Act and in safeguarding activities.
* Participate in multidisciplinary team meetings and contribute to service user reviews.
* Engage in research, audit and quality improvement activities relevant to OT practice.
