The Advanced Nurse Practitioner will provide advanced clinical care including assessment, diagnosis, treatment and management of patients presenting with undifferentiated, acute and longterm conditions. You will undertake independent prescribing, request and interpret investigations, and make appropriate referrals to other services.
The role includes conducting facetoface, telephone and digital consultations, contributing to sameday demand, and supporting continuity of care for complex patients. You will work closely with GPs, nurses, pharmacists and the wider PCN team to deliver coordinated care.You will also contribute to service development, clinical governance, quality improvement and the supervision or mentoring of other staff where appropriate, helping to support learning and best practice across the team.
We are a wellestablished GP practice working within a collaborative Primary Care Network, serving a diverse and supportive local community. Our team includes GPs, ANPs, practice nurses, pharmacists, care navigators and an experienced administrative team who work closely together in a positive and respectful environment.

The Advanced Nurse Practitioner (ANP) will work as an autonomous practitioner within the primary care team, delivering highquality, evidencebased care to patients.
