What will you be doing?
The role of and Transitional Care Worker is varied, you work with clients on a short term basis whilst a package of care is built for them. Your main responsibilities will include:
* Working with clients on a short term basis whilst a suitable package of care is created.
* Encouraging clients to be as independent as reasonably possible.
* Assisting with the administering of medication.
* Following your clients care plan to ensure the best quality care is provided.
* Providing support and companionship to your clients.
* Assisting with personal care.
* Manual handling and using specialist equipment.
* Assisting with food preparation.
We are looking for empathetic individuals who enjoy working with others to join our team. We don't mind if you have not worked in the sector before, we provide full training and a shadowing period for all new starters to ensure you are comfortable and ready to work in community.
We also give you ongoing training, which can include our fully funded NVQ program, qualifying you up to level 5 If you have aspirations to progress, we will support that, a lot of our managers and office team started out working in community and have worked their way up.
