We have an exciting opportunity within our South Cumbria Hospital Social Work Team, for a passionate, enthusiastic person to come and join our dedicated and driven team, following an ambitious programme of change within Adults Social Care, to help us develop and deliver our goals for the future.
Our South Cumbria Hospital Social Work Team works collaboratively with a range of stakeholders across the health and social care system in South Cumbria to provide short term assessment and interventions as well as ensure people are supported to manage risk in a positive way, whilst promoting independent outcomes.
A key function of the Hospital Social Work Team is to ensure people can achieve the best outcomes during and post discharge by working closely with our colleagues in the Transfer of Care Hub and engaging in Multi-Disciplinary Discussions and Early Social Work Interventions to ensure people are discharged from both acute and community hospitals in a safe and timely way.
Who we are looking for…
We are looking for a passionate and motivated individual with experience or awareness of social care, healthcare, or the voluntary sector. You should be empathic and have strong communication skills to collaborate with customers, carers, and stakeholders. A proactive approach to risk management, and health and safety is essential, along with the ability to plan workloads, meet deadlines, and use IT effectively. Most importantly, you should be committed to promoting independence, influencing positive change, and ensuring high-quality, strengths-based care.
As a Social Care Worker within the Hospital Social Work Team, you will be community based, working as part of a Multi-Disciplinary Team undertaking strengths-based assessments and delivering appropriate person-centred support and outcomes for people in Westmorland and Furness, who have care and support needs following discharge from hospital. Consistently working in a way that promotes independence and enables individuals/groups to maximise their strengths.
