Job Summary
Honiton Surgery is looking for an experienced Pharmacist to join our practice team. Based at Honiton Surgery you will work with our GPs and other clinicians to support prescribing and review medication. Whilst working directly for the surgery, you will also work alongside and be supported by our PCN team of pharmacists and pharmacy technicians.

Whilst working directly for the surgery, you will also work alongside and be supported by our PCN team of pharmacists and pharmacy technicians.
Whilst we are advertising for 1 WTE, applications from candidates interested in a part-time role of no less than 0.6WTE are welcome.

Main duties of the job
As Clinical Pharmacist You Will:
See patients with single or multiple medical problems where medicine optimisation is required e.g. diabetes, hypertension, heart failure,
Carry out structured medication reviews for patients identified as priority by the practice or through the PCN DES.
Carry out medicines reconciliations for patients across care boundaries to ensure that our patients get the right medicines in a timely manner with the correct drug monitoring.
Answer prescription and medication queries from patients and staff across the health network.
Present and lead improvement projects on PCN wide policies and procedures.
Contribute to Practice and PCN meetings, GP educational meetings and any other meetings as and when required.
Support the GP prescribing leads with implementing change, maintaining standards and achieving prescribing targets e.g. those set through the Devon Medicines Optimisation Scheme and Impact and Investment Fund.
About Us
We are a large, supportive and friendly well-established GP practice of 18,000 patients and are located in the market town of Honiton in East Devon. We have a diverse clinical team that includes 5 GP partners,13 salaried GPs, advanced nurse practitioners, pharmacists, pharmacy technicians, FCP physiotherapists and occupational therapists. We also have a health and well-being coach team that works closely with our local CMHT. We are the lead practice in our PCN working with two other local practices and have a good rating with CQC. In addition, we are a training practice currently training GP registrars, AHPs, nurses and nurse practitioners.
We are an RCGP Research Ready practice with many active active research studies. The Partners and Practice Manager are innovative and forward thinking, proactive in quality improvement work for developing our service, supporting excellent patient care and staff wellbeing. Successful applicants could be supported to become involved in delivery of research and quality improvement projects

Review the ongoing medicine plan for these patients, ensuring they get the best use of their medicines and make appropriate recommendations to clinicians for medicine improvement

This will be provided by face to face and phone appointments and may also involve visiting people in their own homes or in care homes. Although the candidate does not need to be a non-medical prescriber at time of application, they must be willing to work towards this goal and will be supported to achieve this following completion of the CPPE Primary Care Pharmacist Programme.
* Carry out medicines reconciliations for patients across care boundaries to ensure that our patients get the right medicines in a timely manner with the correct drug monitoring. This will involve liaising with colleagues in secondary care and possibly accessing further support through the community pharmacy discharge medicines scheme or referral to the community services pharmacy teamAnswer prescription and medication queries from patients and staff across the health network.

* Support the delivery of QOF, incentive schemes, QIPP and other quality or cost effectiveness initiatives.
* Provide education and training to primary healthcare team on therapeutics and medicines optimization.
* Work with the general practice team to ensure the practice is compliant with CQC standards where medicines are involved.
* To support public health, including vaccination campaigns. To provide specialist knowledge on all public health programmes available to the general public.
