Community Mental Health Nurse - MHSOP

An exciting opportunity has arisen for a full time Band 6 care co-ordinator to join our team. We are looking for a versatile, motivated, committed and experienced mental health nurse to work with adults over the age of 65 who are living with a functional and/or organic illness.

We are a supportive, experienced and highly motivated multidisciplinary team. The successful candidate would be, committed to work within the recovery model and dedicated to improve the well-being of service users and their carers'. As a member of the team you will manage the holistic needs of people on your case load, undertaking a range of assessments, provide a range of psychosocial interventions. As a care coordinator you will also take responsibility for commissioning care using Local Authority systems and procedure.

Main duties of the job

To deliver evidence based holistic care to clients within the context of a multi- disciplinary, multi-agency Community Mental Health Service.

To take responsibility for a clinical caseload providing comprehensive assessment, care and treatment to clients referred to the Community Mental Health Team in accordance with Parts 2 and 3 of the Mental Health (Wales) Measure 2010.

Foster robust working relationships with all partners and key stakeholders, in particular Local Authority, Primary Care and the Third sector.

Contribute to the continuing improvement and development of community mental health services.

The ability to speak Welsh is desirable for this role; Welsh and/or English speakers are equally welcome to apply.

Person Specification


Qualifications

* Demonstrable post registration development through relevant courses and study days
* Psychological therapies


Experience

* Experience in clinical practice within the area of speciality
* Experience of working withing a multi-agency community mental health setting


Skills

* Organisational ability and time management skills
* Clinical assessment and formulation skills


Special Knowledge

* Demonstable post - registration knowledge through experience as a qualified mental health nurse / attendance of formal / informal courses / training
* Ability to demonstrate sound clinical knowledge and awareness of current evidence base relevant to the area of practice.
* Working knowledge of the legal and policy framework underpinning mental health service provisions
* Knowledge of safeguarding procedures
* Experience of delivering psychosocial interventions / CBT and / or experience of postgraduate psychosocial skills training


Personal Qualities

* Flexible to the service
* Ability to work under pressure
* Ability to work on own initiative


Other

* Ability to travel in a timely manner to undertake the duties of the post at various locations
