Sources of Support is part of the Primary Care Health Inclusion Service which operates in all GP Practices in Dundee. We are part of the National Community Link Worker programme. Our team consists of 8 Primary Care Link Workers and 2 Primary Care Associate Practitioners (Support Workers).
Our remit is to work with patients whose physical and mental health and wellbeing is impacted by social and economic issues which means that the service will offer non-medical interventions and co-ordinated care to help improve the patients’ personal circumstances.
The Primary Care Link Worker works as part of the wider GP practice team, covering up to 3 GP Practices and alleviating pressures on the GP practice of non medical issues experienced by patients
Primary Care Link Workers case manage the needs of the patient for up to 20 weeks to help them achieve their identified goals. The Primary Care Link Worker works alongside the patient, utilising a range of different approaches to affect change and facilitate access to appropriate services with a view to improving physical and mental wellbeing, and quality of life. Advocacy and liaison with primary and secondary care, statutory and third-sector services is a key feature of the Primary Care Link Worker role.
Candidates must be able to demonstrate working at a senior practitioner level, working with people on a one-to-one basis who have complex social, economic and personal circumstances. Candidate should demonstrate their experience in being responsible for managing a complex caseload where highly skilled decision making, liaison with other services, managing risk and autonomous working are required. A robust understanding of the challenges faced by people living in deprivation; and experience of person-centred working and therapeutic interventions are required for this post. As is experience of multidisciplinary and collaborative working with partners within a range of work settings. Applicants must be educated to degree level in a relevant area, preferably with evidence of post-graduate training or qualifications.
You will be required to achieve the mandatory Induction standards for Healthcare Support Workers in Scotland and to comply with the Code of Conduct throughout your employment.
