An opportunity has arisen for an experienced and motivated Pharmacist to provide strategic and clinical leadership to a Realistic Medicine Pharmacy Team within NHS Borders.
This role will lead the delivery and development of high‑quality, person‑centred pharmaceutical care, with a strong focus on polypharmacy reviews, care home medication reviews, and medicines management assessments across primary and community care settings.
The Role
As Lead Pharmacist, you will:
1. Provide clinical and professional leadership to a pharmacy team delivering Realistic Medicine‑led polypharmacy reviews
2. Lead and oversee medication reviews for patients with complex needs, frailty, or multimorbidity
3. Drive the delivery of care home medication reviews, working collaboratively with GP practices, care home staff, and wider health and social care professionals
4. Lead medicines management assessments, ensuring safe, effective, and person‑centred use of medicines
5. Embed Realistic Medicine principles, supporting shared decision‑making and personalised care planning
6. Contribute to service transformation, quality improvement, and implementation of national polypharmacy guidance
7. Ensure clinical governance, prescribing quality, and risk management across services
8. Support education, supervision, and professional development of pharmacists, pharmacy technicians and pharmacy support staff
9. Use data and audit to demonstrate impact, outcomes, and continuous improvement
You Will Bring
10. Registration with the General Pharmaceutical Council (GPhC)
11. Experience in polypharmacy reviews and working as an independent prescriber
12. Experience of working across organisational boundaries in multidisciplinary teams
13. Excellent communication skills and commitment to person‑centred care
14. Knowledge of NHS Scotland polypharmacy guidance and Realistic Medicine principles
15. Experience in service development, quality improvement, or strategic planning would be desirable
