This position offers a sign-on bonus and is located in Falmouth, MA, with a schedule of 37.5 hours per week, Monday through Friday, 8 am to 4 pm, including rotating weekends and holidays.
Responsibilities include:
* Maintaining annual HHRG values at the National Standard.
* Utilizing HHRG values to establish visit frequency and interdisciplinary utilization.
* Meeting National Benchmarks for annual outcomes.
* Referring to community resources as appropriate.
* Evaluating patient needs, developing and revising care plans, documenting accordingly.
* Participating in interdisciplinary case conferences and supervising Home Health Aides and COTAs.
* Providing patient and family education on occupational therapy treatments.
* Completing timely documentation and discharge planning.
* Adhering to departmental guidelines for supervision and case conferencing.
Qualifications:
* Bachelor's or Master's Degree in Occupational Therapy from an accredited institution.
* Current OT license in Massachusetts.
* Minimum one year of clinical experience and CPR certification.
* Preferred computer skills.
