Are you a GP looking to work regular 8 hour shifts including dedicated admin time? We are expanding our GP service and looking for clinicians to work across our 2 practices at East Park Medical Centre and it's branch surgery in Halton.
We are looking for a clinician to work 4-6 sessions a week. With a mixture of 8-4 or 10-6pm shifts, one day needs to be Friday.
We have a multidisciplinary team of clinicians who work together to support each other to deliver GP services to our practice population of just over 10000 patients. The Medical team includes Advanced Nurse Practitioners, Mental health nurse and care navigators.
We are a training practice of medical students and working towards GP registrars and involved with research projects.

About us
Our clinical team is currently made up of a GP clinical Lead, GPs, ANPs, ACPs, Mental health nurse, Pharmacists, Pharmacy technicians, nurses and HCAs. Our non clinical team contains a management team, medical secretaries, prescribing clerks and receptionists.
There are clinical meetings most weeks for all the clinical team to cover clinical governance, safe guarding, palliative care and complex presentations

Job responsibilities
Clinical Responsibilities:
-East ParkMedical Centre will be operating a Tier-Based Delivery of Care, withtheir patients being segmented into 3 Tiers 1, 2 and 3. GPs will be expected tohelp deliver care to each of these tiers on a rota basis.
-Tier1 represents those patientswith the lowest physiological risk factors for illness and no other significantor combinations of chronic health problems. The GP will be part of a multidisciplinaryteam compromised of GPs, Nurse Practitioners, Advanced Care Practitioners and Mental Health nurse. They will have their own appointment schedule whichallows time to support the wider MDT team with the more complex problems andqueries, reviewing these patients, as necessary. The overall clinicalresponsibility for the MDT will rest with the Clinical lead and GPs mustescalate any concerns regarding the team and patient care to the lead in atimely fashion.
-Tier2 represents patients whohave one or more significant chronic health problem or are at a time in theirlife of increased physiological risk for example, pregnancy or older age. Thisis the group of patients where continuity of care can have the greatest impacton patient outcomes and the practice will be striving to achieve this. The GPhere will work alongside the nursing team delivering chronic health management,with their own appointment schedule but also supporting the nursing team whereappropriate. The GP will be involved in liaising with PCN health professionals,community and secondary health regarding patient care.
-Tier3 represents the mostcomplex patients, these patients maybe palliative, represent safeguarding risksor be going through a time of crisis which is impacting on their health. Carehome patients will be included in this Tier and the GP will be expected tosupport the MDT teams and meetings to plan these patients care. These patientsare likely to require the GP at times to perform home visits where remoteconsults are not appropriate.
-Administrativeduties such as issuing of prescriptions,checking pathology results, tasks and incoming letters/documents for the practicewill be performed in dedicated working time in the appointment schedule or a dedicatedsession as part of the rota.
-The GPwith appropriate training may also be asked to participate and hold GroupConsultations.
-The GPwill also be expected to attend the daily team huddles.
In performing the duties above the GP will be expected to:
-In accordance with the Practice timetable, asagreed, the post-holder will make themselves available to undertake a varietyof duties including surgery, telephone, video, home and care home consultations.
-Compiling and issuing computer-generated acuteand repeat prescriptions (avoiding hand-written prescriptions wheneverpossible), checking and signing repeat prescriptions and dealing with queries,paperwork, reports and correspondence in a timely fashion.
-Prescribing in accordance with the Practiceprescribing formulary (or generically) whenever this is clinically appropriate
-Making professional, autonomous decisions inrelation to presenting problems, whether self-referred or referred from otherhealth care workers within the organisation.
-Assessing the health care needs of patients withundifferentiated and undiagnosed problems.
-Screening patients for disease risk factors andearly signs of illness.
-In consultation with patients and in line withcurrent practice disease management protocols, developing care plans forhealth.
-Providing counselling and health education.
-Recording clear and contemporaneous consultationnotes to agreed standards.
-Collecting data for audit purposes and complyingwith QOF requirements.
-Delivering enhanced services.

Qualifications
* MBBS or equivalent medical degree
