Main Responsibilities:
* Provide safe and compassionate care and support that is person-centred.
* Perform personal care tasks as outlined in the client's care plan.
* Create and maintain good communication with clients and other members of the support network.
* Establish and maintain effective working relationships with clients.
* Document, monitor and report any changes to care managers.
* Assess care plans versus actual provision.
* Assess risks and needs, reporting to the care manager.
* Participate actively in clients' care team meetings.
* Adhere to all Eclectic Care Ltd policies and ensure new staff are inducted into clients' care packages and follow procedures.
* Offer companionship and emotional support to enhance the well-being of clients.
Ideal Candidate Must Be Able to Support Clients With:
* Attending appointments and social activities.
* Helping with dressing.
* Laundry and light cleaning.
* Meal preparation.
* Medication management.
* Personal care, including washing and bathing.
* Shopping.
