Overview
The post holder will work as part of a dynamic multi-agency integrated team assessing referrals regarding ICRAS Service requirements for patients across Liverpool and South Sefton.
Responsibilities
* Ensure due regard to patient confidentiality; the coordinator needs to interpret and share the information that is necessary to promote ICRAS across organisations.
* Be responsible for co-ordinating care delivered through the ICRAS model.
* Support the delivery of the ICRAS model and influence service development.
* Operate within a complex health and social care system, across Health and Social Care and third sector providers.
* Provide leadership within ICRAS services.
* Teach and present to groups of staff about changes in ICRAS Service provision and direction and ensure policy change is implemented across the whole system.
* Coordinate interventions for patients, communicate with GP practices and Secondary Care providers, and collaborate with staff inside and outside the ICRAS model.
* Take ownership on behalf of the ICRAS model for continuous quality improvement, innovation and integration.
* Contribute to development and delivery of the quality strategy within the designated area, including establishment of systems and processes.
* To ensure smooth running of respective ICRAS teams, clinically review, coordinate and distribute referrals, directing staff accordingly.
* Manage MDT meetings/SAFER huddles associated preparation and facilitation.
* Coordinate input from Health Professionals outside core/specialist services for patient care or MDT attendance.
* Manage incidents and complaints.
* Ensure the interface between acute hospitals, primary care, community health services, social care, housing, the independent sector, and the voluntary sector is robust at all times.
* Maximise the capacity of ICRAS teams and ensure services are responsive to local needs.
* Engage with other health and social care systems around good practice for ICRAS and research models of service.
* Ensure ICRAS Care providers deliver high quality care appropriate to client needs.
* Ensure effective use of Integrated Care services across the Whole System.
* Support analysts in reporting data, identify information gaps and pressures within the whole ICRAS system.
* Ensure standards of service are delivered in line with local and national guidelines across all professional groups.
* Support line management and authorised signatory of identified accountable staff; support/training and development of ICRAS staff.

* Maintain current professional registration.

* Provide evidence-based practice and high quality integrated care services tailored to the needs of individuals within the ICRAS.
Management
* Maintain constructive relationships with internal and external stakeholders from a wide range of services and disciplines.
* Coordinate complex activities spanning multiple disciplines and organisations (NHS and non-NHS).
* Record, interpret and present health information that can impact risk or needs assessments; disclose information fairly and proportionally.
* Develop and implement data collection systems and ICRAS protocols for accurate and timely data within confidentiality and data protection requirements.
* Disseminate decisions and health information to relevant health professionals for sharing best practice.
* Identify service gaps and issues relating to clinical pathways and cross-boundary arrangements; escalate ineffective arrangements.
* Record health outcomes on appropriate systems on a case-by-case basis.
* Participate in internal and external working groups/projects to provide information and analytical advice.
* Carry out other duties as reasonably required by line manager.
Leadership
* Contribute to performance improvement of the ICRAS and lead for identified areas.
* Provide coordination and participation in working groups; offer advice, expertise and support.
* Provide timely reports on the function of the ICRAS to the Urgent and Intermediate Care service line within Community Division.
* Investigate causes of variance in ICRAS activity and contribute to solutions.
* Lead and participate in groups to achieve targets and objectives for ICRAS.
* Identify risks and issues to facilitate resolution or escalation.
Clinical
* Act as a resource on ICRAS care for other disciplines, agencies and the public.
* Prioritise and manage time/workload; inform line manager of constraints.
* Oversee identified team members' roles, responsibilities and development.
* Provide support and advice within the team in areas of professional expertise.
* Support staff in developing skills within the Scope of Professional Practice.
* Provide cover for absent care coordinators as needed across Liverpool and South Sefton ICRAS teams.
* Follow all policies and procedures; provide clinical leadership as appropriate.
Non Clinical
* Provide regular reporting and monitoring from quality and safety perspectives.
* Contribute to strategic planning of ICRAS team projects and interdependencies across projects, functions and services.
* Contribute to performance and governance strategies and integrated care improvement programmes.
* Contribute to short-, medium- and long-term service developments for ICRAS to achieve quality outcomes.
* Manage own workload with informed decisions in changing timescales.
* Use computer software to create reports for professionals and meetings; develop policies and procedures affecting the wider organisation.
* Contribute to the ICRAS team budget and manage a designated budget and procurement of assets and supplies.
Training & Education
* Participate in annual performance reviews and pursue Continuous Professional Development.
* Utilise learning opportunities both independently and with others.
* Lead coordination and provision of training to ensure understanding of ICRAS.
* Provide internal and external training development for the ICRAS team.
* Practice in accordance with NMC and Mersey Care NHS Foundation Trust protocols and guidelines.
* Be accountable for own practice.
