Offer description

The 10-bed unit will provide holistic care for patients with both malignant and non‑malignant life‑limiting illnesses and their families/carers. This includes excellent symptom control, psycho‑social and spiritual support, specialist palliative rehabilitation and end‑of‑life care. We also aim to care for patients with complex nursing needs such as non‑invasive ventilation and tracheostomy care. We are looking for a highly motivated Health Care Assistant to join our team on the Inpatient Unit.


Responsibilities

* Work within a multidisciplinary team providing clinical care and support to hospice patients and their families.
* Maintain effective communication skills to deliver emotive information clearly, sensitively and unambiguously with staff, patients, families and professionals by face‑to‑face, telephone and/or electronic means.
* Collaborate with the MDT, sharing the clinical caseload of patients/carers within the locality/in‑patient unit to ensure efficient and timely care is delivered.
* Seek advice from colleagues and/or clinical leadership when required to deliver excellent palliative and end‑of‑life care.
* Support and participate in streamlined locality/IPU processes and services, working to agreed quality improvement standards and outcome measures.
* Build strong relationships with external partners and stakeholders, promoting the good reputation of the Hospice (e.g. local trusts, CCG, health and social care partners, businesses and voluntary organisations).
* Ensure the service delivers an excellent standard of care to patients and their families.
* Actively promote patient and family/carer involvement in care.
* Operate under the direction of the Locality Care Team Manager, undertaking agreed clinical activities with designated patients.
* Monitor patients for any changes in symptoms and provide non‑pharmacological relief where possible, referring for professional advice when required.
* Provide personal care, pressure area care and mouth care as per the personal care plan.
* Be flexible and creative in the care of the patient and family, involving and enabling family and friends to continue caring for the patient and support them as appropriate.
* Provide emotional support and communicate effectively with patients and their family/carers.
* Work unsupervised in the patient’s home as required.


Essential Qualifications

* Good level of general education.
* NVQ Level 2.
* Passionate about hospice care and enthusiasm for the vision of the organisation.
* Honest, organised and reliable.
* Maintains confidentiality of patients and their families.
* Team player who can work as part of our One Team ethos.
* Flexible approach to work, with ability to work out of hours on occasions.
* Committed to working in partnership with people, their relatives and carers and other organisations and agencies.


Desirable Experience

* Experience in a palliative or end‑of‑life care setting.


Essential Experience

* Previous experience of working in personal care.


Essential Skills / Ability / Knowledge

* Ability to empathise with the needs of clients.
* Ability to communicate effectively, both verbally and in writing, with a diverse client group.
* Ability to work alone and in a team.
* Ability to work under pressure and in difficult/emotional situations.
* Good understanding of English language spoken and written.
* Good IT skills and ability to input information onto computer systems.


Other Requirements

* Driving licence and car ownership (community).
* Satisfactory Criminal Records Disclosure.
