We are seeking to recruit an enthusiastic, highly motivated Band 5 Occupational Therapist to work within our Integrated Neighbourhood Team (INT) in North & West Bristol.

The Sirona Integrated Neighbourhood Teams (INT) are an innovative and dynamic service. We provide active rehabilitation to patients in their place of residence to maximise their functional potential and support them to remain independent within their homes. Our therapists support hospital discharges, admission avoidance, and long-term self-management, enabling patients to live better lives in their home environment.
You will be working within a friendly and supportive INT alongside other Therapists, Assistant Practitioners, Nurses, Nurse Associates, Advanced Clinical Practitioners, Community Support Workers, and Administrators.
We offer a flexible service which covers extended hours operating 7 days a week.
Main duties of the job
* With the support of senior staff, be responsible for the initial holistic assessment, implementation, and evaluation of programmes of care for patients who are acutely and chronically unwell, often with complex needs arising from the frail elderly cohort.
* Use clinical reasoning skills to assess, plan, implement, and evaluate patient-centred intervention programmes using an evidence-based practice approach, promoting high-quality and harm-free care.
* Collaborate and negotiate with service users, carers, and family in identifying achievable goals as part of the overall care plan, which will maximise quality of life and independence.
* Facilitate appropriate referrals to other services.
* Work within a multidisciplinary team, sharing knowledge and providing specialist skills supported by a robust competency framework.
* Use occupational therapy skills and techniques in functional and standardised assessments and interventions as an autonomous practitioner.
* Assess and arrange the provision of equipment, ensuring it is provided in good working order and fitted correctly; report any issues appropriately.
* Supervise team members, including peers, students, and unregistered workforce, in occupational therapy competencies.
* Maintain accurate, informative, and legal records.
* Actively participate in training; this may involve assisting in the delivery of training.

Person Specification
Qualifications
* BSc or equivalent in Occupational Therapy & HCPC registered.
* Evidence of continuing professional development and commitment to lifelong learning.
Skills, Knowledge & Experience
* Working knowledge of OT outcome measures.
* Effective communication skills—written, verbal, and non-verbal—with the ability to adapt communication for individuals with communication difficulties.
* Able to make clinically reasoned judgments when working with various conditions.
* Able to work autonomously and know when to seek advice from senior staff.
* Effective time and caseload management.
* Community work experience (student or postgraduate) demonstrating a broad range of clinical skills.
* Knowledge of health, safety, and risk management policies.
* Experience working with a range of professionals in health or social care.
* Up-to-date knowledge of Evidence-Based Practice, local, and national standards.

Salary: £31,049 to £37,796 per annum, pro rata
