Highly Specialist Physiotherapist Band 7 – Full Time Permanent Post

(Part time hours will be considered)

We are delighted to offer a new Physiotherapist post within our expanding and innovative Frailty at Home Service in Dundee. This role has been created alongside a new Occupational Therapist role, strengthening our multidisciplinary team and supporting our ambition to deliver high‑quality, proactive care to older adults living with frailty in the community.

About the Service

Frailty at Home is a forward‑thinking service that works to identify deterioration early, support rehabilitation at home, and provide alternatives to hospital admission. Our approach is proactive, person‑centred, and focused on enabling independence and wellbeing.

About the Role

As a key member of the team, you will:

* Provide specialist physiotherapy assessment and intervention for adults living with frailty
* Work collaboratively with Occupational Therapy, Nursing, Medicine, Pharmacy, Social Care, and other partners
* Participate in early intervention, rehabilitation planning, and holistic support at home
* Contribute to smoother transitions of care, including supported discharge and preventing unnecessary hospital admissions
