What You Will Do
Carry out comprehensive assessments of patients in the community, identifying needs and developing personalised care plans.
Work closely with a multidisciplinary team to ensure coordinated, high-quality support and improved outcomes for patients.
Deliver evidence-based interventions designed to promote independence and reduce reliance on formal care services.
Monitor and review progress, adjusting care plans as needed to ensure the best possible results.
Provide advice and education to patients and families, empowering them to manage their health and wellbeing.
Requirements
HCPC registration as an Occupational Therapist.
Experience in community-based practice, ideally within Adult Social Care.
Valid driver’s licence to facilitate travel within the Liskeard community.
