Job responsibilities The PCN Lead Clinical Pharmacist acts within their professional boundaries to shape the delivery of medicines optimisation within the PCN. In addition to the usual responsibilities of a PCN Pharmacist, the Lead PCN Pharmacist will be a key member of the Medicines Optimisation Strategy Group, leading on the prioritisation and allocation of medicines work to achieve QoF, DES and LES contracts. They will lead on the development of shared operating practices, procedures and safe working practices and provide supervision to the PCN medicines team. They will act strategically to ensure the team improves patients health outcomes and by providing direct, accessible and timely medicines and expertise including face to face contact with patients. The post holder will be an integral part of the general practice teams, working with the other PCN medicines roles in each practice and across the PCN, as well as part of a wider pharmacist network and multidisciplinary team. This includes being a conduit of patient medicines information into and out of local hospitals. They will work to optimise medication issues to improve patient care and safety and support clinical staff in the management of patients. We are looking for a dynamic pharmacist leader who relishes a challenge and who can bring their energy and innovation to further develop clinical pharmacy within our PCN. This role will involve being part of the PCN multi-disciplinary team and requires you to provide expertise in clinical medicines management.
