Salary: £28,000 FTE
Hours: Full time – 35 hours per week
Days and Times: Monday to Friday, 9am – 5pm

ACCESS TO A CAR AND A CLEAN FULL DRIVING LICENCE IS A REQUIREMENT OF THIS POSITION
We are looking for an experienced person to work with older people being discharged from hospital to ensure there is an efficient pathway for discharge to take place, by reducing the likelihood of experiencing a fall and/or readmission to hospital, promoting independent living and removing barriers to daily living activities.
