Role Overview Join NHS Greater Glasgow and Clyde as a Band 6 Care Home Liaison Nurse within the West Dunbarton Older Adult CMHT, based at Goldenhill Resource Centre in Clydebank. This rewarding role places you at the heart of community mental health care, supporting older adults living within care home settings. You will deliver specialist mental health nursing advice, assessment and care coordination, helping to improve quality of life and prevent unnecessary hospital admissions. Working within a compassionate, multi‑disciplinary environment, you will build strong partnerships across services to deliver person‑centred care.
Duration: Full Time, Fixed Term
Location: Goldenhill Resource Centre
Shift Pattern: Monday‑Thursday 9 am‑4:45 pm / Friday 9 am‑4:30 pm
Key Responsibilities
* Provide specialist mental health assessment, diagnosis and care planning for older adults in care homes
* Deliver expert advice, support and education to care home staff, carers and families
* Manage and prioritise your own caseload, ensuring effective and timely interventions
* Liaise closely with GPs, community services and multidisciplinary teams to coordinate care packages
* Monitor and review care plans regularly to maintain independence and prevent hospital admission
* Support hospital discharge pathways and facilitate safe, timely returns to care home settings
* Maintain accurate records and ensure clear communication between care homes and community services
Required Knowledge, Training, Qualifications and Experience
* First level registered mental health nurse with significant post‑registration experience in a community setting
* Evidence of continuing professional development and relevant post‑registration learning
* Strong knowledge of older adult mental health conditions and therapeutic approaches
* Excellent communication, organisational and interpersonal skills
* Ability to work autonomously and as part of a multidisciplinary team
* Working knowledge of relevant mental health and community care legislation
* Full driving licence and basic IT skills
