Two Rivers Medical Centre has a fantastic opportunity for an experienced practice nurse to join our team for a 12 month fixed term contract, on a part-time basis for 25 hours per week.
Main duties of the job
* Undertaking health assessments, administering medication and treatments, and performing diagnostic tests.
* Providing and evaluating a range of health interventions to ensure that patients health and social care needs are met.
* General health and well-being promotion as well as creating individual, person-centred plans for patients with, or at risk of, a long term condition.

Person Specification
Qualifications
* Current NMC registration with an active PIN.
* Fundamentals of Practice Nursing course (General Practice Nurse Foundation Programme) or equivalent experience or qualifications
* Competence to undertake asthma, COPD and diabetic clinics and reviews, holding relevant certificates.
Experience
* Evidence of clinical skills and experience in treatment room consultation, phlebotomy, ECG recording, screening, management of long-term conditions, cervical cytology sampling, vaccinations and immunisations, assisting with minor surgery, dressing and wound care, and recording spirometry.
* Compassionate manner with an empathetic approach and demonstrable kindness in all aspects of your work. The successful candidate will value the dignity of our patients, their relatives and carers, as well as their colleagues at the practice.
* Knowledge, and experience of its application, in supporting long-term conditions.
* Insight and understanding of current issues in nursing and the wider NHS.
