Job Overview
There has never been a more exciting time to work in primary care on the Isle of Wight, and Newport Health Centre is looking for a Clinical Pharmacist to join the team on a fixed term/maternity cover basis. We are seeking a positive, caring and self-motivated individual to take on the role of Clinical Pharmacist. About the Role
Location: Newport Health Centre, 22 Carisbrooke High Street, Newport, Isle of Wight, PO30 1NR
Salary: £54,000 per year pro rata
Contract: Fixed term, 12 months
Working pattern: Full-time, Part-time, Flexible working
Responsibilities
* Working as part of a multi-disciplinary team in a patient-facing role to clinically assess and treat patients within your competence, using your medicines expertise for specific disease areas
* Managing care for patients with chronic diseases and undertaking clinical medication reviews to proactively manage people with complex polypharmacy
* Providing specialist expertise in the use of medicines while addressing public health and social care needs and tackling inequalities
* Supporting the leadership team in medicines optimisation and quality improvement, in line with local antimicrobial stewardship guidance
* Supporting patients to take their medications correctly, reduce waste and promote self-care where possible
* Supporting clinical aspects of shared care protocols, clinical research with medicines, liaison with specialist pharmacists and community pharmacists
* Being part of a professional clinical network with access to clinical supervision
Qualifications and Experience
* Essential: Degree/Master in Pharmacy (or equivalent); GPhC registration
* Desirable: Royal Pharmaceutical Society faculty membership; independent prescribing or working towards prescribing qualification
* Essential: In-depth therapeutic and clinical knowledge and experience of evidence-based healthcare; multi-disciplinary working
* Desirable: Experience delivering patient care in primary care, urgent or community settings
Skills and Personal Qualities
* Excellent communication and interpersonal skills; ability to liaise with multi-disciplinary teams
* Holistic assessment and ability to critically analyse patient information
* Strong IT skills; proficiency in time management and prioritisation
* Ability to work both as part of a team and autonomously; high integrity and resilience
* Ability to work under pressure and maintain confidentiality
