Greater Manchester ICB- Bury operates a Continuing Healthcare/Complex Case service on behalf of the registered population of Bury. The post holder will be responsible for undertaking Specialist Health Needs assessments for individuals who are undergoing assessment for, or who are already eligible for NHS Continuing Healthcare and NHS Funded Nursing Care. The assessments may take place in an individual's own home, residential placements, or hospital environments.
Responsibilities include:
1. Conducting health needs assessments for individuals with complex healthcare needs.
2. Working with individuals, their representatives, private providers, partner organizations, and other health and social care professionals to ensure person-centered assessments.
3. Supporting the 'Duty' rota to manage urgent referrals and safeguarding investigations.
4. Promoting service user involvement and collaborative working with other agencies.
5. Providing guidance and supervision to staff undertaking clinical assessments.
6. Conducting independent expert clinical assessments when necessary.
7. Supporting the Head of Service in identifying individuals eligible for funding and liaising with care providers.
8. Supporting Adult Safeguarding strategy meetings.
9. Ensuring reviews of service users eligible for health funding are conducted in accordance with guidance.
10. Maintaining accurate records and assessments.
