Lead Support Worker role at TieTalent. Location: Lymington, Hampshire. Permanent, full time (37.5 hpw), rota working. Pay £13.21 per hour (salary £25,838 per annum) with benefits including a Health Cash Plan.

As our Lead Support Worker, you’ll be at the heart of supporting customers who have complex mental health needs to develop their skills to live more independently in the community. You’ll make a big difference each day as you help our customers achieve their hopes and aspirations.
Typical day as a Lead Support Worker (known by us as Coordinator).
Responsibilities
* Creating person-centred support plans and risk management plans with your customers.
* Helping customers with daily living activities such as making meals, shopping, budgeting, maintaining their tenancy and moving onto independent accommodation if able.
* Working with our Health Coach to support customers to achieve positive health outcomes, or with our training officer to help access training, education and employment.
* Carrying out risk assessments, support planning, goal setting and regular reviews.
* Cuppas, chats and catch ups. Plenty of time to collaborate with colleagues.
You bring
* Passion to support customers to live their best life, with attention to detail and collaborative working.
* Experience of creating person-centred support plans and supporting colleagues to implement them.
* Experience of coordinating and assessing customer referrals.
* Ability to work on your own initiative, stay calm under pressure and have a resilient approach.
* To travel between locations, a vehicle insured for business purposes is required; mileage paid.

Team and environment
You’ll join a large Hampshire West Team based at Lymington in the New Forest with some travel to team meetings and training; involvement with outreach work in the local area is possible. The management team includes Emma, Karen, Ade and Lucy, and the team is passionate about providing daily support to customers and supporting each other.
Job details
* Rolling rota pattern including 9am-5pm, 10am-6pm, and 11am-7pm shifts with some weekends and bank holidays.
* Flexibility to meet customer needs, which may require adjusting start or finish times occasionally.
* Occasional lone work, with regular daytime training and team meetings.
* Ability to use technology for creating/updating support plans, completing online learning, and collaborating with colleagues.
* Enhanced DBS check required; we pay for this.
