ThePharmacy Technician will work within North Cornwall Coast PCN Practices, todeliver audits, medicines reconciliation and general medicines optimisationtasks under the supervision of the PCN Pharmacy Team
Thepost holder will work closely with the teams to help and support themulti-disciplinary team. This will include on-going patient case management.This will involve working with the GP surgeries and linking in with a range ofcommunity health and social care services, care homes and third-party services
Thepost holder will support those at home to live in the least restrictive way andthose in care homes to remain listened to and their voices heard
Thepost holder will demonstrate excellent organisational and communication skills,be flexible in their approach, able to exercise initiative, and demonstrateconsistently high standards of professionalism
Thepost holder will support the PCN team with administrative tasks such as lettercoding and EMIS tasks
Thepost holder must always be aware of the need for confidentiality and integrity
Main duties of the job
Completeaudits including medication safety and prescribing and supporting the practiceswith QOF targets
Carryout medicines optimisation tasks including effective medicine administratione.g., checking inhaler techniques, supporting medication reviews and medicinesreconciliation, along with high-risk drug monitoring
Supportmedication reviews and reconciliation for care home patients and synchronisingmedicines for patient transfers between care settings
Supportthe Pharmacist in Structured Medication Reviews
Manageshared care protocols and liaise with the PCN Pharmacist for more complexpatients
Supportinitiatives for antimicrobial stewardship to reduce inappropriate antibioticprescribing locally
Providetraining and support on the legal, safe handling of medicines, including theimplementation of EPS
Promotionof Electronic Repeat Dispensing and online ordering
Supportthe implementation of national prescribing policies and guidance within GPpractices, and community
Workwith GPs and other primary care professionals within the PCN and/or practicesto identify and manage a caseload of patients
Raiseawareness within the PCN of shared decision making and decision support tools
