Are you acompassionate and experienced Registered Nurse with a passion forpalliative care? Do you thrive in managing complex caseloads and deliveringhigh-quality care in patients homes? St Nicholas Hospice Care isseeking a Community Hospice Specialist Nurse to join our dedicatedteam supporting patients across West Suffolk and Thetford.

The rolecovers the core hours 9-5pm, with a rotational weekend day cover.


Main duties of the job

As a Community Hospice Specialist Nurse, you will:

* Manage a complex caseload of patients with life-limiting conditions, providing expert assessment, symptom management, and care planning.
* Deliver person-centred care in community settings, including patients homes, care homes, and clinics.
* Work autonomously while collaborating with Clinical Nurse Specialists, GPs, and other healthcare professionals.
* Provide clinical leadership, mentoring, and support to junior staff and Health Care Assistants.
* Offer emotional and psychological support to patients and families, ensuring dignity and choice at every stage.

Job responsibilities

* Participate in the provision of individualised specialist palliative and end of life care for patients in their own home.
* Demonstrate sound clinical decision making skills and increasing autonomy in practice.
* With support, manage a caseload of patients with palliative and end of life care needs.
* Support junior colleagues to develop clinical expertise.
* Equip patients, families, carers and healthcare professionals with the skills and knowledge to support palliative and end of life care, through formal and informal education.
* Act as a role model, demonstrating the role of Junior Clinical Nurse Specialist.
* Actively support and model the SNHC Values: Compassion, Accountability, Respect and Equity.

Person Specification


Qualifications

* NMC Registered Nurse with significant experience in palliative or community nursing.


Experience

* Strong clinical decision-making skills and confidence in managing complex symptoms.
* Excellent communication and interpersonal skills.
* Ability to work independently and as part of a multidisciplinary team.
* A commitment to co-creating care with patients and families.
