Job summary
You will assist the Operational Lead with the management of CRHT whilst also working collaboratively with the other clinical leads. You will work pro-actively with the operational lead to ensure safe effective delivery of services and developments relevant to the teams. Your key focus will be to support patients in private beds/ out of area placements and to help to facilitate early discharges from the wards. You will support with unblocking any barriers to discharge.
You will be support in the safe and effective management of all operations in the team.
You will be required to work closely with the Operations lead to develop the teams in line with commissioner requirements and Trust objectives, consulting all key stakeholders, and positively reinforcing the expectations of the organization through collaborative and dynamic management and leadership that is both supportive and encouraging in nature to enable the team to adapt to key changes smoothly.
The post holder will provide day-to-day clinical leadership and co-ordination to a multi-disciplinary, psychiatric intensive Crisis Resolution Home Treatment Team. They will provide visible daily leadership and clinical supervision to a team multidisciplinary staff. They will work to ensure that the service provided is of best value and well organized. They will deputize for the Operational Lead in their absence.
Main duties
* Assist the Operational Lead with the management of CRHT and work collaboratively with other clinical leads.
* Support patients in private beds/out of area placements and facilitate early discharges from the wards.
* Unblock any barriers to discharge and support early discharge assessments from out of area wards.
* Lead on improving the quality and performance of the teams through QI projects.
* Improve experience of care by effectively delivering services to support service users, carers and partner organisations.
* Focus on discharges from private beds to support patient flow and unblock barriers.
* Improve population health outcomes by leading the team with holistic assessments.
* Improve staff experiences including timely rota finalisation, leading team and business meetings, facilitating away days and ensuring team takes annual leave as per policy.
* Improve value of the service and increase efficiency.
* Assist the operational manager to ensure that the physical environment is conducive to delivering a high quality of care.
* Maintain clinical direction and leadership; coordinate a team of clinical staff for the continuous and consistent provision of high quality mental health care to people in mental health crisis in the community.
* Carry continuing responsibility for the assessment of care needs, the development, implementation and evaluation of care programmes and the setting of standards of care for clients of the service.
* Demonstrate clinical leadership and knowledge of a range of therapeutic interventions available to reduce distress for individuals experiencing an acute mental health crisis.
* Act as a role‑model in the practice area, encouraging staff in teaching, supervision, facilitation and assessment, and create and sustain a supportive/teaching/learning environment for the practice setting.
* Be responsible for the assessment, development, implementation and evaluation of programmes of care for patients, incorporating risk assessment to ensure safe and acceptable standards of care.
* Maintain and accurately record patient information in the documentation systems and ensure the effective dissemination of patient information throughout the multi‑disciplinary team.
