The East Lancashire Alliance is currently recruiting a nurse associate on behalf of the Hyndburn Central PCN.
This is a full time position for 24 months.
Main duties of the job
Be responsible for delivering high quality, patient centred, compassionate care under the direction of a Community Link Nurse with a focus on promoting health and independence.
Be an accountable professional.
Work as part of a designated clinical and care team delivering care that focuses on the direct needs of the individual.
Carry out specific clinical and care tasks and responsibilities to a high standard and competency, under the direction of a Community Link Nurse.
Have the ability to work without direct supervision, at times delivering care independently in line with the individuals defined plan of care, within the parameters of practice of the Nurse Associate role, accessing clinical and care advice when needed.

Clinical responsibilities

The Nurse Associate will:
Developunderstanding of all elements of the nursing process and be able to assist the Community Link Nurse in the on-goingassessment, planning, management and evaluation of care
Provide and monitorcare
Support individuals with all aspects of careincluding daily living, providing person-centred care and promotinghealth and independence through awareness raising andcare navigation to other professionals as appropriate
Perform and record clinical observationsincluding blood pressure, temperature,respirations, pulse and NEWS
Accurately record nutritional and fluid intake
Ensure the privacy,dignity and safetyof individuals is maintained at all times
Demonstrate the ability to recognise changingpriorities seeking advice andguidance from the Registered Nurse or other registered care professionalsas appropriate
Report back and share information with the registered nurses on the condition, behaviour, activity and responses of individuals
Recognise issues relating to safeguarding adults and report any problems or raise concernsto the appropriate registered care professionals
Assist in the assessment of and contribute tothe management of risk across severalareas within the environment where care is being administered
Assist in the implementation of appropriate action to meet the specific physical, emotional andpsychological, social, cultural and spiritual needs ofindividuals and carers
Assist in the delivery of care as prescribed by the Community Link Nurse

Qualifications
* Registered Nurse Associate on the NMC register
* Care Certificate
* Educated to a minimum level 5 foundation degree specific to nursing associate practice
* GCSE level 4-9 in Maths and English or equivalent
