Healthcare Assistant required to join our team in Cardiff Community providing compassionate, high-quality end-of-life care. You will be part of a multidisciplinary team delivering holistic, person-centred end-of-life care under the guidance of registered professionals, supporting patients and their loved ones with personal care, symptom management and emotional support.
Main duties of the job
You will contribute to care planning, assist with daily living activities, and help maintain a safe and dignified environment. This is a compassionate, hands-on role where you will make a meaningful difference in people\'s lives during their final stages, while supporting their loved ones through grief and bereavement.

What we\'re looking for
* NVQ Level 2 in Health and Social Care or equivalent Level 2 qualification (e.g. NCFE, BTEC), plus functional skills in numeracy and literacy at Level 2
* At least 6 months of recent care delivery experience in a health or social care setting
* Strong communication skills with the ability to hold person-centred, empathetic conversations and work effectively within a multidisciplinary team
* Knowledge of palliative and end-of-life care, including holistic care planning and symptom management
* Ability to plan and prioritise care, respond to changes in patient condition, and escalate appropriately
* A valid driving licence and access to a vehicle
