Responsibilities
* Manage a team of colleagues to help people gain more independence in daily lives.
* Develop and implement person‑centred support plans.
* Conduct care needs assessments.
* Recruit and induct new colleagues.
* Complete supervisions/appraisals.
* Complete medication competencies and medication audits.
* Attend multi‑disciplinary meetings.
* Prepare reports and assist the Registered Manager in day‑to‑day management.
* Participate in the on‑call rota (subject to change depending on service needs).
Qualifications
* Previous experience in a care management role.
* Level 3 in Health & Social Care.
* Experience working with individuals with autism, acquired brain injury, complex mental health needs, traumatic backgrounds, and challenging behaviours.
