Job Summary
An interesting and exciting opportunity has arisen for a highly motivated and enthusiastic individual to join Cancer Services at Nottingham University hospitals as a Cancer Pathway Coordinator.As the Patient Pathway Coordinator, the successful candidates will play a key role in helping with the delivery of cancer targets for the trust by ensuring that the journey of each cancer patient runs smoothly and is progressed in a seamless and timely manner.
Job Summary

A visit to the department prior to application/interview is highly recommended.
* Please note this post is for 2 x 12 month fixed term posts at 30 hours per week **
Main duties of the job

The post holder will be responsible for tracking cancer patients in line with national cancer targets and will facilitate data collection for suspected/ diagnosed cancer patients, entering the information onto appropriate database(s). The post holder will need to be flexible and self-motivated with a good attention to detail and the initiative to work independently as well as a team. This post is key in providing high quality information and ensuring patient's progress effectively on the cancer pathway.The post holder will work closely with staff across the wider trust including clinicians, general managers, deputy managers, nurses, secretaries and other administrative staff.
Ideally the post holder will have previous experience working within the NHS and previous administrative experience with an understanding of cancer plans, standards and targets.

Cancer Pathway Co-ordination
Using hospital systems, proactively manage the patient pathway for all tumour sites by monitoring, reporting and recording tracking events, ensuring that the patients journey is processed in a timely and efficient manner, in line with the Cancer Waiting Times (CWT) Targets.
To retrieve information needed to maintain a tracking process on every two-week wait patient, consultant upgrade and screening referral.
Work with Cancer Pathway Managers to ensure the pathway reflects the requirements agreed for each tumour site timeline.
Provide weekly updates to the Cancer Pathway management team and the wider divisions regarding the progress of individual patients in the diagnostics stage, escalating issues and delays in a patients pathway as identified by the Cancer PTL.
To gain an understanding of the medical terminology of diagnosis and treatments for cancer in order to fully understand the patients pathway.
Ensure effective communication with nursing staff, medical teams, management and other coordinators to ensure the organisation and coordination of timely management of all patients on a cancer pathway.
Send and receive tertiary referrals and investigation / treatments results for patients from other provider organisations in line with local agreements and national targets.
Data Collection and Analysis
Ensuring data collection is undertaken in a prospective manner, assuring accuracy at all times.
Analysing target breaches and reporting any anomalies to the Cancer Pathway Managers and Head of Performance.
Work with clinical teams to assist in the collection and recording of items for Nation Cancer Clinical data sets e.g. Cancer waiting times, COSD, Clinical audits etc.
