CJP Outreach Services are expanding across our South Region, and we are currently seeking a compassionate, dynamic, and experienced Case Manager. This pivotal leadership role will provide support to multiple teams across a range of different supported living settings. The role reports directly to the Registered Manager for the South. You will be working closely with the people we support, their families, and multidisciplinary teams to deliver high quality support and improve quality of life outcomes using a positive behaviour support framework. This role also includes supervising team leaders, ensuring compliance with both national and local policy and maintaining best practice. Key Responsibilities * Support the Registered Manager and other Case Managers in all aspects of running the South Region. * Lead and manage an agreed caseload of supported living packages, ensuring the highest standards of person-centred care. * Support your teams to implement Positive Behaviour Support (PBS) strategies to promote positive outcomes, improve quality of life, and reduce behaviours of concern. * Supervise and support staff, ensuring they have the necessary skills and knowledge to deliver high-quality support. * Act as the primary point of contact for the team leaders, staff, the people we support and their families, providing responsive action in addressing concerns. * Monitor staff performance, conducting supervisions, mentoring, and addressing performance improvements where needed. * Regularly visit your caseload packages to assess satisfaction, quality, and support delivery. * Support your team leaders to organise and manage staff rotas. * Conduct quality audits on your caseload packages, recommending and implementing improvements as needed. * Promote and protect the welfare of the people we support, maintaining effective working relationships within the team. * Support the Registered Manager to develop and update care plans, and other related documentation. * Support with recruitment and induction of staff. * Monitor absences and holidays, arranging suitable cover when needed. * Participate in the local on-call rota and provide hands-on support when necessary. * Participate in personal and professional growth opportunities, including training and supervision of staff. * Attend required training sessions, team meeting and other such professional meetings. Essential Criteria * Previous experience in a similar position (for example deputy manager or Team Leader) within the learning disability and/or autism sector. * Hold (or be working towards) a QCF Level 4 Diploma in Leadership for Health and Social Care (or equivalent). * Strong knowledge of CQC standards, safeguarding protocols and the principle of the Mental Capacity Act. * Strong knowledge of Positive Behaviour Support. * Experience in supported living services for people with learning disabilities, autism and mental health needs. * Experience working with individuals transitioning from secure hospital services to community settings. * Excellent communication, leadership, organisational and problem-solving skills. * Proficiency with IT systems and care management reporting tools. * Full, clean UK driving licence and willingness to travel across the South Region. * Willing to undergo an Enhanced DBS check.
